1) This permission slip must be signed in order to participate in Extracurricular sports at CMS. Students who had already turned in a sheet before the Cougars Soccer or Lady Cougars Soccer seasons, do not need to have it signed, as we already have a copy.

2) Students must also pay a $20 Athletic fee and have all other fees up to date (student fee paid in full, no long-overdue library books, etc.) in order to participate in Extracurricular sports at CMS.
